Each participant receives individual attention to help them with their medical, physical, cognitive, mental, and/or emotional needs. Our caring professional team develops an individual plan of care with each participant and their doctor. This plan becomes our tool in helping participants to grow as individuals emotionally and to achieve the highest level of physical and cognitive functioning possible.
Our participants have a greater opportunity to feel better, maintain their independence, and reduce the chances of hospitalization or unwanted placement in an institutionalized setting.
Caring for a loved one with medical, physical, cognitive, mental, and/or emotional problems can be exhausting. The ADHC program allows the family and caregiver time to work, have a break, and still continue to care for their loved one.
